Coverage under the University benefit plans will terminate as of the last day of the month in which your employment terminates, you cease to be in a class of employees eligible for coverage, you fail to make the required contribution, or the plan is terminated.
Dependent Coverage terminates as of the last day of the month in which the dependent loses eligibility or on the last day of the calendar month when the employee’s insurance terminates, whichever occurs first.
Medical, Dental, & Health Care Flexible Spending Account - Insurance Extension (COBRA)
Under federal law (COBRA), Colorado State University is required to notify you of your right to continue coverage under "The Plan", which includes group health, dental, and/or a health flexible spending reimbursement account, when the coverage would otherwise end because of specific qualifying events described below. This notice is intended to inform you of your rights and obligations under the continuation provisions of the law.
Federal Regulations do not require employers to offer continuation of coverage to Domestic Partners or to the “qualifying” dependents of the domestic partner. Colorado State University has elected to extend COBRA benefits to Domestic Partners and their “qualifying” dependents.

Basic Term Life/AD&D
The policy may be “converted” to an individual age-rated policy within 31 days of termination. Contact the United of Omaha Life Insurance Company at (800) 826-8054 for more information regarding conversion.
Voluntary Group Term Life Insurance
The policy is “portable” in that you can continue at the same group rates if you apply and pay for the first month’s premium within 31 days of termination of coverage. The policy may also be “converted” to an individual age-rated policy within 31 days of termination of coverage. This conversion may be continude up to age 70.
